In Home Services at CHS
The CHS In Home Services Program provides a variety of offerings to individuals with disabilities living in the community. The services help people retain housing in their communities as long as possible. The services include light housekeeping, basic personal care assistance, chore service, meal preparation, completion of simple errands/shopping, motivation and instruction, and social activities.
CHS Hoarding Pilot Program
The CHS Hoarding Pilot Program works with people who have a hoarding disorder to assess the situation, create a plan to remove items, improve the conditions in the home, and work on behaviors to help make sure that each participant can remain independent in their own home.
Health Station at CHS The CHS
Health Station is a nurse-based community clinic that serves individuals participating in CHS programs, neighbors and visitors to the Lawn Street Community Center. Visitors can see the nurse, ask questions about health conditions and monitor their vital signs. The Health Station provides assistance with chronic disease management, such as diabetes and heart disease, to members of the community. The nurse is available on-site two days a week and can consult with interested patients by appointment. There is a monthly ask the nurse session, and the Health Station provides a podiatry clinic two times per month at no cost to patients.
